A 39-year-old woman with HIV presents to clinic for follow-up. She is pregnant and at 36 weeks’ gestation. She is taking antiretroviral therapy and 8 weeks ago her HIV RNA was undetectable. At this visit, however, she reports difficulty with adherence taking the antiretroviral medications due to worsening depression. Laboratory studies are ordered, including an HIV RNA level. She is referred to psychiatry for evaluation, counseling, and potential medication management.
