A 28-year-old woman who is pregnant tests positive for HIV on a screening test done in the first trimester of pregnancy. She is urgently referred to initiate antiretroviral therapy. Initial laboratory studies showed a CD4 count of 718 cells/mm³, an HIV RNA level of 31,510 copies/mL, and a negative HLA-B*5701 test. She has not previously received long-acting injectable cabotegravir. Genotypic drug resistance testing has been ordered and results are pending.
