A 34-year-old woman with HIV presents to the clinic after recently learning she is pregnant. She is likely in the first trimester. She has a history of prior virologic failure while taking tenofovir DF-emtricitabine-cobicistat-elvitegravir, and prior HIV drug resistance genotypic testing showed resistance to lamivudine (and emtricitabine) and integrase inhibitors. Her regimen was changed to tenofovir alafenamide-emtricitabine plus darunavir 800 mg once daily and ritonavir 100 mg once daily. The HIV RNA levels have been undetectable for the past year. She is tolerating this regimen well and would prefer not to change her regimen during pregnancy.
